Obesity paradox in chronic heart failure with moderately reduced or preserved left ventricular ejection fraction: impact on a prognosis for patients according to a five-year follow-up

چکیده

Aim. To investigate the obesity paradox by studying impact of overweight and abdominal on prognosis in chronic heart failure (CHF) patients with moderately reduced or preserved left ventricular ejection fraction (LVEF) five-year follow-up results. Materials methods. A prospective, open, parallel-group study included 314 CHF LVEF, average age was 65.3 ± 11.2 years. Group 1 – 66 normal weight; 2 67 patients; 3 90 I degree obesity; 4 91 subjects II–III degrees obesity. Adverse cardiovascular events (CVEs) were studied as a cumulative endpoint, death, rehospitalization due to decompensated CHF. Results. It has been found that excess body weight (overweight I–III obesity) did not affect prevalence spectrum adverse CVEs LVEF. In patients, compared group, incidence end point (by 15.4 %; χ2 = 3.95, p < 0.05) cases re-hospitalization 19.0 5.6, more often observed. Concomitant associated an increased risk for faster onset (HR 1.46, 95 % CI 1.1–2.2, 1.53, 1.1–2.4, 0.05). Conclusions. The presence LVEF during follow-up. overweight, comparison obesity, disease decompensation observed, which could be evidence existence paradox. most effect caused concomitant which, there significant increase disease.

Secondary Mitral Regurgitation in Heart Failure with Reduced or Preserved Left Ventricular Ejection Fraction

Secondary mitral regurgitation (MR) has been extensively studied in heart failure due to reduced ejection fraction. In contrast, the occurrence and the pathogenesis of secondary MR are much less known in heart failure with preserved ejection fraction (HFpEF). The present review aimed at describing this common but ignored feature of HFpEF. Copyright © 2013 S. Karger AG, Basel As in patients with...

Effects of exercise on left ventricular systolic and diastolic properties in patients with heart failure and a preserved ejection fraction versus heart failure and a reduced ejection fraction.

BACKGROUND The purpose of the current study was to define exercise-induced changes in indices of left ventricular (LV) systolic and diastolic properties in patients with chronic heart failure (HF), compare these changes in patients with HF and a reduced ejection fraction (EF) versus HF and a preserved EF, and compare the hemodynamic responses to activities of daily living with symptom-limited u...
